Sciatica vs Piriformis Syndrome – What’s the Difference?
Both conditions involve the sciatic nerve, the largest and longest nerve in the body — roughly the width of your little finger. It runs from your lower back, through the buttocks, and down each leg.
Because of its length and position, compression can occur at several points, leading to pain and discomfort.
What Is Sciatica?
Sciatica occurs when the sciatic nerve is compressed — often by a slipped disc or vertebral issue.
This can cause pain that radiates from the lower back down one leg (sometimes to the foot).
It can feel:
- Sharp or shooting
- Burning or electric
- Like a deep throbbing ache in the buttock or leg
Severe cases may require surgical intervention to relieve the compression.
What Is Piriformis Syndrome?
Piriformis syndrome occurs when the piriformis muscle (in the buttock) spasms or tightens, irritating or compressing the sciatic nerve.
This can result from:
- Muscle imbalance (overactive or underactive glutes)
- Prolonged sitting (e.g., desk jobs or driving)
In around 20% of people, the sciatic nerve runs through the piriformis muscle, making compression more likely.
Fortunately, exercise and physical therapy can usually help ease symptoms.
When Massage or Manipulation May Not Help
- Bone spurs (osteoarthritis)
- Severe disc degeneration
- Spinal stenosis (narrowing of the spinal canal)
In these cases, medical or surgical options may be required.

- The pain is caused by a herniated or slipped disc
- The sacroiliac joint (where the spine meets the pelvis) is inflamed
- You’re experiencing Piriformis syndrome
